We are going to give you information and invite you to be part of this research study carried out by MemTrax LLC. You can participate in this study by taking the MemTrax Memory Test while you are signed in to your registered MemTrax account.

Before you decide to take part it is important that you understand why the research is taking place and what your participation will involves.

Purpose of Research

Alzheimer’s Disease is the sixth leading cause of death in the United States. More than 5 million Americans are living with the Alzheimer’s Disease, and 1 in 3 seniors dies with Alzheimer’s or another type of dementia. Early identification through regular screening is key to effective treatment. The best way to identify early memory loss is to see if memory is changing over time. The screening methods that are currently used to help detect memory related problems are not as effective as we believe they can be. In fact, most available tests are expansive, time-intensive, and need to be administered by health care professionals. The MemTrax Memory Test is a free, easy to use, short, fun, and researched-based memory screening test. The reason we are doing this research is to further validate the MemTrax Memory Test. The findings of this research will contribute to a greater understanding of effective memory screening.


If you participate in this research, there may not be any direct benefit for you. However, your participation may help to advance the techniques used to assess memory and may contribute to a greater understanding of what constitutes effective memory screening.


The research is completely anonymous and confidential, and handled in accordance with the Data Protection Act 1998. Responses will be recorded with a participant number and it will not be possible for you to be identified in any reports of the data that is gathered.

Voluntary Participation

Your participation in this research is entirely voluntary. It is your choice whether to participate or not.

Right to Refuse and Withdraw

Whether you choose to participate or not, you will still be able to use all services on If you decided that you no longer want to be a part of this study, you are free to withdraw at any time without giving any reason.
